Even in cases where the villain appears to be beautiful at first, there is usually a transformation to reveal that this person is bad. It's like people are afraid of the concept of not being able to determine whether someone is evil just by looking at them - it frightens people that they cannot immediately recognize danger. Even in relatively friendly tales (Beauty and the Beast), the reversion from hideous to beautiful accompanies a shift in personality from negative to positive.
